Untuk melakukan reset password Penyedia, Anda dapat menyampaikan permohonan reset password beserta dokumen pendukung ke alamat email helpdesk.spse@pu.go.id :
1. Surat permohonan perubahan Reset Password dengan Kop Surat Perusahaan dan bermaterai 10000 di Tanda Tangani Oleh Direktur/Direksi
2. KTP direksi/direktur/pemilik perusahaan/pejabat yang berwenang di perusahaan
3. NPWP Perusahaan
4. SIUP/SIUJK/NIB/Ijin untuk menjalankan kegiatan/usaha sesuai Bidangnya dan ketentuan perundangan yang berlaku
5. Akta pendirian perusahaan, serta akta perubahan terakhir (jika ada)
6. Profile Perusahaan (khusus foreign vendor)
Permohonan reset password harus disampaikan dengan menggunakan email yang telah terdaftar.
To reset the Provider's password, you can submit a password reset request along with supporting documents to the email address helpdesk.spse@pu.go.id:
1. Application letter for Reset Password change with Company Letterhead and stamped 10000 signed by the Director/Director
2. ID card of the directors/directors/company owners/authorized officials in the company
3. Tax ID
4. SIUP/SIUJK/NIB/Permit to carry out activities/business according to the field and the provisions of the applicable laws
5. Deed of establishment of the company, as well as deed of latest amendment (if any)
6. Company Profile (foreign vendors only)
The password reset request must be submitted using the registered email.
